Gudbinji Chronic Conditions Care
Wurli established this separate chronic disease clinic to provide a culturally appropriate and supportive service and education with an emphasis on self-management of chronic disease conditions. Coordination of the different services required by individual patients is a big feature of Gudbinji’s work and client care is not limited to clinical care or referral to allied health care. More time is used to provide client case management, education, support and advocacy to ensure a holistic approach to chronic disease management.
The Program places very strong emphasis on developing preventative strategies revolving around healthy eating and exercise promotion and education.

The Gudbinji Chronic Disease Clinic will look after you if you are an adult who has been diagnosed with a chronic disease such as:

- heart disease
- rheumatic heart disease
- respiratory disease
- renal disease
- cancer

We have a reception area and a peaceful garden where you can wait.
How to see us
You can come to the Gudbinji reception at 6 Kintore Street during our operating hours and ask to see a health care practitioner
You can phone 89729157 and ask to be picked up and brought in to see our health care staff
We will send out reminder letters and messages to you when you or your children are due for an appointment or other type of follow up care
